Review of Fuchs's Radiographic Exposure and Quality Control (Fuch's Radiographic Exposure Processing & Quality Control) (Hardcover)

This book is written for a non-technical audience and provides a collection of rules of thumb based on practical experience but not on rigorous understanding of fundamentals. Many simplifications go too far and some explanations are outright misleading. By reading this book, a radiology tech may glean a few tips to produce a good radiograph on most systems. Yet the book may lead to fallacious intuitions. This book should not be used to gain accurate practical understanding of the physics and system design principles in radiography.

The practical rules in the book will probably work fairly well in most cases for conventional systems. New digital imaging systems may render even the practical tips obsolete since they are based on practical experience with conventional systems and not on fundamentals.

Here are some of the most egregious fallacies:

Page 10 states that radiographic images are formed thanks to the polychromatic nature of the x-ray beam: "If all of the x-rays were of the same energy, the image would be extremely poor, essentially a silhouette." In reality, the opposite is true -- a monochromatic beam with all photons carrying the same energy would provide the best image quality (tissue contrast) with best patient dose efficiency (at appropriate keV). Tissue differentiation is produced by differential attenuation of photons in tissues, even if all the photons carry identical energies. X-ray system designers go to great lengths to reduce the polychromaticity of the x-ray beam.

Page 59 states that with the doubling of the number of atoms in some space, the likelihood of x-rays hitting an atom when passing through the space also doubles. Wait a minute! What if the original likelihood of hitting an atom was 70%? Will that probability go to 140% when the number of atoms is doubled in that space? Of course, this description is incorrect. Doubling the number of atoms in a given volume will square the likelihood of the x-ray photon passing through the volume.

On page 129, the author states that "Excessive filtration in the beam defeats the purpose of exposing the radiograph. When density losses become visible, they necessitate an increase in the mAs to compensate. Such an increase in mAs simply reintroduces additional patient skin dose, which the filters were added to eliminate. ...an optimum level of protective filtration has been establishedto achieve maximum level of protection for the patient without affective the radopgrahic density." In reality, no such optimal level can be established. Increasing the filtration will always reduce skin dose from a polychromatic source (x-ray tube) even after the mAs is increased to maintain proper film exposure. Additional spectral filtration equivalent to 2.5 mm of aluminum is simply required by safety regulations. Anything above that is an improvement. The only limitation to adding more spectral filtration is the increased x-ray tube loading and the tube's limited maximum output. This error is particularly disturbing because it cause one to miss opportunities to reduce patient dose while improving image quality when extra mAs can be afforded or to choose to purchase a system that is less dose efficient.

The book's lack of rigor leads to the example on page 154 which states that "bone stops 40 x-rays for every x-ray stopped by soft tissue." In reality, the linear attenuation coefficient of bone is at most 2 or 3 times higher than that of soft tissue at typical x-ray energies.

Product Description:
This new Seventh Edition is a most expansive and comprehensive text on radiographic exposure and imaging and encompasses the vast and intricate changes that have taken place in the field. As with previous editions, the book is intended to complement radiographic physics texts rather than duplicate them. It bridges the gap between theory and practice, and therefore assumes some basic knowledge of physical principles upon which the concepts of practical technique can be built. This volume also attempts to bridge the gap between quality control and technique. Part I, Producing the Radiographic Image, presents chapters on x-rays and radiographic variables, recording the permanent image, qualities of the image, and interactions of x-rays within the patient. Part II, Visibility Factors, includes chapters on milliampere-seconds, kilovoltage-peak, machine phase and rectification, beam filtration, field size limitation, patient status and contrast agents, pathology and casts, scattered radiation and image fog, grids, intensifying screens, and image receptor systems. Part III, Geometrical Factors, discusses focal spot size, the anode bevel, source-image receptor distance, object-image receptor distance, distance ratios, beam-part-film-alignment, geometric functions of positioning, and motion. Part IV, Comprehensive Technique, presents chapters on analyzing the radiographic image, simplifying and standardizing technique, technique by proportional anatomy, technique charts, exposure controls, patient dose, quality control, and solving multiple technique problems. Part V, Special Imaging Methods, includes mobile radiography, special imaging techniques, fluoroscopic image intensification, digital imaging, and computed radiography. Part VI, Processing the Radiograph, completes the text with chapters on processing steps and chemistry, developmental variables, automatic processors, film handling and duplication procedures, and sensitometry and darkroom control. Each chapter concludes with an examination that will help the student review materials and put them into perspective. Multiple choice, fill-in-the-blank, and identification/explanation questions are all included. An answer section is provided at the back of the text. This book is by far the the best available for schools that are focused on the practical application of radiographic technique. Its greatest strengths are its clear and concise explanations, plentiful examples, and abundant illustrations.

Review of The Rat Brain in Stereotaxic Coordinates, Fourth Edition (Paperback)

Paxinos & Watson's rat brain atlas is the most cited and used brain atlas in research labs, and it deserves that status. However, in a couple places it may be misleading or confusing. This is particularly evident in the brainstem. We have found irregularities in the ventrolateral medulla (both caudal and rostral) in this book (co-labeled as C1 and A1), as well as an apparent mix-up between the obex and calamus scriptorius of the rat. However, in the grand scheme of things, this atlas is a great tool to have. Also, the vector graphics data provided on the CD is an INVALUABLE tool when preparing figures.

Review of Essential Clinical Anatomy (Point (Lippincott Williams & Wilkins)) (Paperback)

I still have yet to find the ultimate text for use with anatomy. The Moore volume is probably the best to use as your main text when taking either undergrad anatomy or gross. As with any anatomy text - you will always need supplemental resources - esp. if you are taking gross anatomy. The selection that I had always found useful in the past was the triple combo of Moore's text, Grant's dissector, and Netter's Atlas. With these three you have essentially everything you need for taking gross anatomy - the embryology texts that are usually used for gross anatomy vary, but often contain the same exact info - I feel that is a more individual choice.
I find the Moore text to be very concise - drawings are clear and well labled - good discussion questions at the end of chapters - and the book is very well laid out, esp. in the always intense "head and neck".I highly recommend this volume.

Review of Dog Breeds: An Illustrated Guide (Paperback)

I initially saw this book at my Veterinarian's office in San Diego and was very impressed by it. I took down the publisher's name and address

During my last visit there with my two Keeshonden, I was disappointed to learn that it had been thrown away due to other customers' overuse/abuse?!

The Vet Techs also said that when they tried to reorder it, they learned that it had been discontinued. Utilizing my previous information, I tracked it down to Amazon and was delighted to be able to order it at a really good price.

This beautifully drawn and printed book now resides on my family room cocktail table to be enjoyed by all my guests.

Thank you Amazon.

Review of Surgical Anatomy Around the Orbit: The System of Zones
A Continuation of Surgical Anatomy of the Orbit by Barry M. Zide and Glenn W. Jelks (Hardcover)

This book is an outstanding surgical anatomy atlas. The fresh-frozen cadavers are beautifully dissected, discussions are thoughtful, illustrations are well used, and the section on blocks is very helpful. The chapters on the lower eyelid and the facial nerve are truly outstanding. Together with the forehead anatomy book by Knize, these two books belong on the shelf of every oculofacial plastic surgeon. The CD with photos and illustrations is a nice bonus. Overall, I highly recommend it.

Review of Acland's DVD Atlas of Human Anatomy, DVD 5: The Head and Neck, Part 2: Disc 5 (DVD-ROM)

This video prosection of the head and neck is a beautifully prepared, expertly produced aid to the study of anatomy. Having done a dissection course in anatomy, I could not and did not perform as expert a dissection as this video series does. You see cross sectional anatomy that is seldom presented in a formal anatomy lab. Your three dimensional understanding of anatomy will take shape as you watch! Bob Acland, in his soothing British accent, narrates the video and makes you feel as if you're sitting in your living room, in front of the fireplace, sipping a cup of Darjeeling tea! Dr. Acland made it a priority in his life to produce an educational tool for students of anatomy that was affordable on a student's budget. The entire series can be had for far less than most single textbooks!
The series is a pleasure to watch, and there is plenty to refresh the memory of one who has used anatomy daily for over 30 years, as I have. Congratulations, Bob, on a job well done.

Review of Color Atlas of Anatomy: A Photographic Study of the Human Body (Hardcover)

One of the first things that one realizes during anatomy lab is that the paintings in your textbook don't really reflect the reality of an embalmed corpse. Arteries are not conveniently painted bright red, nor are nerves colored a nice polite yellow.
The Color Atlas of Anatomy does a fantastic job of helping one translate the color drawings from the big anatomy textbooks into the lab by providing high-quality labelled photos of model dissections by expert anatomists. Think you have the hypogastric nerve in your abdomenal wall but aren't too sure what it's supposed to look like? My partner and I were in just that position and the Color Atlas helped us go from the idealized material of our lecture notes and Netter's Atlas to realities of our cadaver.
In addition to the photos, I found the schematic drawings to be a nice way to keep in mind the general organization of basic membranes and organs in the body, and the MR and X-Ray scans were useful as well in learning how to read radiograms and MRI images. This book does a great job of teaching you what anatomic specimens really look like, and help you appreciate the great beauty and elegance of the human body.

Review of Systems and Structures: The World's Best Anatomical Charts (The World's Best Anatomical Chart Series) (Spiral-bound)

Again with this particular collection of charts I do not feel as though they covered all that was expected. However, the detail of the charts that were included is quite nice. I believe that there were several books available by this company that may have included a bit more. They also cost more but I think that they may be worth the extra few dollars for the full store of charts.

Review of An Illustrated Adventure in Human Anatomy: Blueprints for Health, 2nd Edition (Paperback)

We got a copy of this book following a visit to the "Bodies Exhibition". My six year old specifically chose this book at the gift shop instead of another that was designed for children his age and younger. When I asked him why, he told me that the illustrations were more realistic and detailed, and there was a lot more information he could soak up (he reads quite well). The writing is accessible and straightforward. My son pulls this book out of the shelf regularly, and continues to do so months after we bought it. Given my son's continued interest in human anatomy and in this book in particular, I must say that this is an excellent reference and has been the launching point for numerous discussions about human anatomy. My son highly recommends it, and so do I.
